Edited from MAIB official report 25/2014. The MARS archive contains many incidents and accidents where the OOW is the sole lookout in darkness. Yet it is an implicit requirement within STCW to keep a separate dedicated lookout on the bridge, in addition to the watchkeeper, during the hours of darkness and in busy shipping areas while underway.
